Copy of birth certificate (needed for new players/cheerleaders only)
Current physical dated in 2009 (physical must be on a state approved form)
Current photograph of player/cheerleader (wallet sized photo)
Signed acknowledgment of Parent's Handbook (available onsite at registration events)
